Daily Self-Monitoring Screening Tool

PART 1

Has the non-fully vaccinated student been in contact with anyone who has test positive for COVID-19?    
YES  /  NO

Has the student been diagnosed with COVID-19 in the past two weeks, OR if they are on a close contact shortened quarantine period have they completed the full number of days?  
YES  /  NO

Has the student developed any of the following symptoms in the past 24 hours? 
YES  /  NO  
  • Cough 
  • Shortness of breath/trouble breathing 
  • New loss or sense of taste or smell 
  • Taken medicine to lower temperature 

IF ANY RESPONSE IS “YES” THE STUDENT SHOULD STAY HOME. Call the school nurse and update on the symptoms for guidance.  
 

Part 2

Has the student had any of these symptoms, new or different from what they usually have, the last 24 hours? 
YES  /  NO   
  • Sore throat
  • Unusual fatigue
  • Nausea (feels sick to stomach)
  • Vomiting *
  • Runny nose or congestion
  • Headache
  • Muscle or body aches
  • Fever of 100.4 OR chills OR taken fever reducing medications *
  • Diarrhea * 
IF “YES” TO 2 or MORE QUESTIONS, THE STUDENTS SHOULD STAY HOME, BE TESTED FOR COVID-19, AND ISOLATE UNTIL RESULTS ARE BACK.

NOTIFY your primary care physician and the school nurse if the test results are positive.  

*Note: Students with fever, vomiting, diarrhea – alone or together- should never attend school. 
** Note: The school nurse may make a testing recommendation given any combination of symptoms.
