Meeting Minutes

1)      Opening Remarks (Bill Ognacevic)

a.       DSHA has mandated that “Event Notice Form” to be submitted for approval prior to any meeting using DSHA facilities.  (Form distributed)

b.      “PR/Marketing Work Request Form” to be used to develop marketing materials including:  flyer; invitation; postcards; other marketing collateral materials.  To be submitted to Toni Letizia a minimum of 14 day prior to need. (Form distributed)

c.       Mailings to be coordinated through Tracy Wayson

d.      Request for volunteer secretary to take notes during meeting

e.       New DSHA website launched:  www.DSHA.info  ;  content still being developed and released;  Parent Association will have separate link;  content to be maintained via Parent Association

2)      Treasurer Report (Jim Podewils)

a.       Parent Association will no longer have a separate check book after 8/30/04.  All Parent Association expenses will be paid by DSHA and require a “Purchase Order” request forms to be competed and approved prior to any expenditures.  Approvals via Jim Podewils or Bill Ognacevic.  Expenses to be submitted to Treasurer, with approve Purchase Order for reimbursement.  (Form distributed)

                                                               i.      Minor expenses and “sundry supplies” will be reimbursed via a check reimbursement form

b.      DSHA Certificate of Tax Exempt Status to be used for all DSHA Parent Association purchases.  Certificate Number ES0054.  (Form distributed)

c.       Requests for “Tax Deduction Receipts” for expenses that are donated to DSHA can be obtained via DSHA administration.

3)      Parents & Students of Color Picnic 8/22/04 (Sheila & Larry Ashley & Arlisia McHenry)

a.       19 families registered and expect 50-70 total attendees;  Ice Breaker events discussed; Event T-Shirt displayed

b.      Post Card invitation distributed

c.       Invitation to Parent Association Task Force Members to participate

4)      Wine & Cheese Social 9/1/04 (Karen Knauf)

a.       Event planned and on track.  Wine & Cheese is ordered.  Coffee will be donated via Starbucks on 124th.  Cheese will be donated by Sendiks on North.

b.      Entertainment “Who’s on First” to be provided in Theater (Ognacevic/O’Boyle)

c.       Parent Association Task Force groups will have separate tables set up for each of our sponsored activities.  Posters will be made for tables.

d.      Post Card invitation reviewed.  Will include RSVP to email address.  

e.       Requested digital pictures to be taken

5)      Parent/Daughter Service 9/12, ~10/7, 10/10 (Linda Goeden)

a.       Events planned and on track

6)      Sub & Substance 10/6,  10/20(Ed & Sheila Fabiszak)

a.       Freshmen and Sophomore Event updates. Agenda: light dinner; panel discussion; moderated break out groups; summary

                                                               i.      Suggested that couple-attendees select different break out groups to facilitate better discussions

b.      Suggested Questions & Event Evaluation Forms provided from prior years.
